  • STUDENT MUST provide a detailed description, including when, how, and all else applicable. Please specify all physical damage or software issues in detail. PLEASE BE AWARE THAT IF A DETAILED DESCRIPTION IS NOT PROVIDED, THIS WILL DELAY YOUR REQUEST.
  • Loaner Chromebook Policy

  • You are submitting a repair request for your school assigned chromebook. If inventory allows, a loaner will be provided to you as we initiate and process a claim. Signing this form acknowledges your understanding that the loaner, when provided, must be maintained in a like-new condition, to avoid a charge of up to $400. The loaner must be returned when the original repaired chromebook is received by the student.
    Please refer to the Chromebook Usage Policy Agreement regarding proper care for the device.

    STUDENTS SHOULD MAKE THEIR APPOINTMENTS DURING THE FOLLOWING TIMES: BEFORE SCHOOL, HOMEROOM, STUDY OR LUNCH PERIODS. CLASS TIME SHOULD NOT BE MISSED!
